Below is a checklist of items you’ll need during registration. Once you’re ready, please take a few minutes to complete your profile using our SureLC program for licensing.

Please have the following available in PDF format:

Copies of trainings and certifications that apply

  • Long-Term Care Insurance (LTCI) – Copies of your original 8 hr. LTC training & current 4 hr. LTC training
  • Medicare – AHIP or equivalent certification (if applicable)
  • Medicaid Compliant Annuity (MCA) – Please click here for state requirements

Date AML training last completed, and name or certificate

*If you do not have AML training, just check “NONE”

  • A voided check
  • E&O Certificate
